Is an HS code required for UPS international shipments?

For any commercial shipment, yes — UPS requires it on the commercial invoice and transmits it as advance data to destination customs. Without it, UPS brokerage classifies the goods on your behalf, which tends toward conservative (higher-duty) codes, and you carry the liability as importer of record either way.

What is UPS Paperless Invoice and does it change the HS code?

Paperless Invoice is UPS's electronic customs documentation — the equivalent of FedEx ETD. It changes how the invoice is transmitted, not what it must contain: the same accurate 6-digit HS code is required per line item. Enrolling in Paperless Invoice with wrong codes just delivers the wrong classification faster.

Why did UPS bill me brokerage fees on top of duty?

UPS charges brokerage and disbursement fees separately from duty and tax, and they are especially visible on shipments into Canada.
